Carers:

You may have been caring for someone for some time already, regularly helping them with everyday tasks or giving them the sort of support they need to stay in the family home.

You may see it as a part of your life or your duty to care for your Mum or Dad, your partner, your child or friend, but there may be times when you need information, advice or some extra help.

When you are a carer it is often difficult to have a real break because someone depends on you to look after them. You can get tired and run down, and your health may possibly suffer.

Telling the Surgery can help us to support you and make sure that you yourself get the right sort of care.

To register your self with us either as a carer, please complete a carer registration form and send or bring it to the surgery.

Access to medical records

Please note that we will always refuse to disclose any information about the person you care for unless they have given written consent for us to do so. 

If you are a carer and the person you care for wants to give consent to you having access to some or all of their medical information, please download a copy of the consent form here.

Carers assessments

Carers are entitled to request a carers assessment from Social Services. You can self-refer by calling 01933 220700. For more information on Carers Assessments, follow the link.

Carer’s allowance is a benefit specifically for carers. It is available to people who care for someone else for at least 35 hours per week and who are not in full time employment, if the person they are caring for receives some of the above benefits. Carers on a low income may also be able to receive a top up to Income Support, called Carer Premium, if they satisfy many of the conditions for Carer’s Allowance.
