We are delighted to share that Albany House Medical Centre, in collaboration with other local practices, has been chosen as a local innovator to spearhead the Neighbourhood Access Service and Complex Care and Long-Term Conditions Service — a pioneering local initiative designed to strengthen primary and community care.
What is a Neighbourhood?
Neighbourhoods are local geographic areas where health and care service work collaboratively to support people in their communities.
When does it start?
Although we’re in the early stages, the service is scheduled to launch in April 2026, supported by the 3Sixty Federation.
More information for patients will be published as we approach the launch of this groundbreaking new service.
Neighbourhood Access Service
What does this mean for patients?
The Neighbourhood Access Service aims to improve access to care and to allow GPs more time to focus on preventing illness rather than reacting to it. This new approach will help deliver faster, more convenient care for everyone.
How will it work?
- Same-day appointments for patients with one-off illnesses or symptoms.
- You’ll still be seen by a GP or an Advanced Practitioner, ensuring high-quality care.
Complex Care and Long Term Conditions Service
What does this mean for patients?
Additional services for patients with Complex Care and Long-Term Conditions are being designed to aid people who need ongoing support for serious or long-term health issues, with the aim to increase accessibility, continuity and holistic care for our populations who need it most, with GPs, nurses, social care, mental health teams, voluntary, community and social enterprise organisations and others coming together to provide joined-up support.
How will it work?
- Regular clinics to offer dedicated consultations in a format best suited to the patient and condition (face to face, telephone, online)
- Multidisciplinary approach, ensuring you are seen by the most qualified professionals.
